Phantom School - 2F Chilly Hallway.
Phantom School Annex - 1F Hallway.
Phantom School 1-4.
Those are the bugged maps I've checked out for Phantom School. Being that getting to the entire 2F is impossible I assume every room in 2F Hallway is bugged and would pretty much say 3F is as well. Same for Annex - 1F Hallway rooms.
